About The Position

Signify Health, part of CVS Health, is seeking a full-time physician to provide in-home health evaluations and help bridge gaps in care. Physicians visit people in their homes to build trusted relationships and deliver flexible, comprehensive, and personalized health evaluations. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and, when ordered by the person’s health plan, additional diagnostic tests such as diabetic eye exams or spirometry. Physicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or change a person’s current treatment regimen. The role involves local travel and travel within licensed states as needed, as well as virtual visits when needed. The primary workplace is the private residence of health plan members. Homes may contain household pets, secondhand smoke, or other airborne allergens or irritants, and candidates must be able to perform their duties comfortably and safely in these environments. The position is full-time, with 40 anticipated weekly hours. The typical pay range is $125.00 per hour.

Requirements

Active, unrestricted physician license(s) in the coverage area(s).
Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
Perform additional diagnostic tests when ordered by a person’s health plan, including services such as spirometry, diabetic eye exams, and bone density screening.
Use an iPad and the clinical workflow for each evaluation.
Travel locally and within licensed states as needed.
Conduct virtual visits as needed.
Provide all indicated Signify Health services, including Diagnostic & Preventive Services products.
Build trusted relationships with people and help connect them to care suited to their needs.
Work as a clinician colleague and fulfill the requirements of the specified role.
