About The Position

Signify Health, part of CVS Health, is seeking a part-time Physician to provide in-home health evaluations and help bridge gaps in care. Physicians conduct visits of about 45 minutes in members’ homes, building trusted relationships and offering personalized health evaluations. Each visit includes a medication and medical history review and a physical evaluation, with additional diagnostic tests when ordered by the member’s health plan. Physicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or change a member’s current treatment regimen. The role may also involve virtual visits and local travel within licensed states. The primary work environment is members’ private residences, which may contain household pets, secondhand smoke, or other airborne allergens or irritants. The position is part time, with 29.5 anticipated weekly hours.

Requirements

• Active, unrestricted medical license(s) in the coverage area(s).
• Ability to comfortably and safely perform duties in members’ homes, which may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Perform additional diagnostic and preventive services when indicated, such as spirometry, diabetic eye exams, and bone density screenings.
• Use an iPad and the clinical workflow for each evaluation.
• Conduct virtual visits as needed.
• Travel locally and within licensed states as needed.
• Provide Signify Health services as indicated and fulfill the requirements of the specified clinician role.
