About The Position

Signify Health, part of CVS Health, is seeking a full-time physician to provide in-home health evaluations and help bridge gaps in care. Physicians visit people in their homes to build trusted relationships and provide flexible, comprehensive, personalized health evaluations. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and, when ordered by the person’s health plan, additional diagnostic tests such as diabetic eye exams or spirometry. Physicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or change a person’s current treatment regimen. The primary workplace is the private residence of health plan members. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants. The role may also involve local travel and travel within licensed states, as needed, and virtual visits as needed.

Requirements

Active, unrestricted physician license(s) in coverage area(s).
Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Multi-state licenses are preferred.

Responsibilities

Conduct in-home health evaluations and build trusted relationships with health plan members.
Review each person’s medication and medical history and conduct a physical evaluation.
Perform additional diagnostic and preventive services when indicated, such as spirometry, diabetic eye exams, and bone density screening.
Use an iPad and the clinical workflow for each evaluation.
Travel locally and within licensed states as needed.
Conduct virtual visits as needed.
Provide Signify Health services as indicated, including Diagnostic & Preventive Services products.
Fulfill the requirements of the specified clinician role and contribute to efforts to improve health outcomes and connect people to appropriate care.
