About The Position

Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations to health plan members. Visits take about 45 minutes and include a medication and medical history review, a physical evaluation, time to answer health questions, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or change a member’s current treatment regimen. The role may also include virtual visits and requires local travel and travel within licensed states as needed. The primary workplace is members’ private residences. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants. Clinicians must be able to perform their duties comfortably and safely in these settings. Full-time employees are eligible for a comprehensive benefits package, subject to eligibility.

Requirements

Active, unrestricted license(s) in the coverage area(s).
Board certification.
Ability to comfortably and safely perform duties in homes that may contain pets, secondhand smoke, and other airborne allergens or irritants.

Responsibilities

Conduct in-home health evaluations and build trusted relationships with health plan members.
Review members’ medication and medical history and perform a physical evaluation.
Answer members’ health questions and provide education and guidance.
Perform additional diagnostic and preventive services when indicated, such as spirometry, diabetic eye exams, and bone density screening.
Conduct virtual visits as needed.
Travel locally and within licensed states as needed.
Fulfill the requirements of the clinician role and provide Signify Health services as indicated.
