In-Home Nurse Practitioner or Physician Assistant Full Time - Charlottesville
About The Position
Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to provide In-Home Health Evaluations and help bridge gaps in care. Clinicians visit health plan members in their homes to build trusted relationships and provide flexible, comprehensive, and personalized health evaluations. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the person’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or alter a person’s current treatment regimen. The primary workplace is the private residence of health plan members. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants. The clinician must be able to perform duties comfortably and safely in these conditions. The position may also involve local and in-state travel and virtual visits as needed.
Requirements
- Active, unrestricted license(s) in the coverage area(s).
- Board certification.
- Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other airborne allergens or irritants.

Responsibilities
- Conduct In-Home Health Evaluations and build trusted relationships with health plan members.
- Review members’ medication and medical history and perform physical evaluations.
- Provide additional diagnostic and preventive services as indicated, such as spirometry, diabetic eye exams, and bone density screening.
- Use an iPad and the clinical workflow to complete evaluations.
- Travel locally and within licensed states as needed.
- Conduct virtual visits as needed.
- Fulfill the requirements of the specified clinician role and provide Signify Health services as indicated.
