About The Position

Signify Health, part of CVS Health, is seeking a per diem physician to provide in-home health evaluations. Physicians visit health plan members in their homes, build trusted relationships, answer health questions, and help bridge gaps in care. Visits last approximately 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the person’s health plan. Physicians use an iPad and a clinical workflow. They are not required to prescribe medicine, order lab tests, or alter a person’s current treatment regimen.

The role involves local travel and travel within licensed states as needed, as well as virtual visits when needed. The primary workplace is the private residence of health plan members. Home environments vary and may include household pets, secondhand smoke, and other potential airborne allergens or irritants; clinicians must be able to perform their duties comfortably and safely in these conditions.

Per diem employees receive an hourly base rate and may be eligible for additional incentives, such as payment per completed visit. Malpractice insurance coverage applies during Signify Health-related activities. This part-time position is eligible for a range of benefits and programs, depending on hours worked and eligibility.

Requirements

• Hold an active, unrestricted physician license in the coverage area(s).
• Be able to comfortably and safely perform duties in home environments that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations and engage with members about their health.
• Review members’ medication and medical histories and perform physical evaluations.
• Conduct additional diagnostic tests when ordered by a member’s health plan, including services such as spirometry, diabetic eye exams, and bone density screening.
• Use an iPad and the clinical workflow for each evaluation.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide Signify Health services, including indicated Diagnostic & Preventive Services products.
• Help connect members with appropriate care and support improved health outcomes.
