In-Home Physician Full Time - Kokomo-LaFayette
About The Position
Signify Health, part of CVS Health, is seeking a full-time physician to provide in-home health evaluations for health plan members in the Kokomo-LaFayette area. The evaluations typically last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Physicians use an iPad and a clinical workflow and are not required to prescribe medication, order lab tests, or change a member’s current treatment regimen. The role may also include virtual visits and locally or state-wide travel as needed. The primary workplace is members’ private residences, which may contain household pets, secondhand smoke, or other airborne allergens or irritants.
Requirements
- Active, unrestricted medical license(s) in the coverage area(s).
- Ability to comfortably and safely perform duties in members’ homes, which may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities
- Conduct in-home health evaluations and build trusted relationships with health plan members.
- Review members’ medications and medical histories and perform physical evaluations.
- Provide additional Diagnostic & Preventive Services when indicated, such as spirometry, diabetic eye exams, and bone density screening.
- Conduct virtual visits as needed.
- Travel locally and within licensed states as needed.
- Fulfill the requirements of the specified clinician role and provide Signify Health services as indicated.
