About The Position

Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide In-Home Health Evaluations in members’ homes and help bridge gaps in care. Visits last about 45 minutes and include time to build rapport, answer health questions, review medication and medical history, conduct a physical evaluation, and perform additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or alter a person’s current treatment regimen.

The role involves serving members in their homes, with virtual visits as needed. The primary workplace is the private residence of health plan members; home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants. Clinicians must be able to perform duties comfortably and safely in these environments. Local travel and travel within licensed states may be needed.

This part-time position has an anticipated schedule of 29.5 weekly hours. The typical pay range is $37.66–$81.11. Actual compensation depends on factors including experience, education, geography, and other relevant factors. Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.
• Ability to comfortably and safely perform duties in home environments that may include household pets, secondhand smoke, and other airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Answer members’ health questions and provide time for one-on-one engagement and education.
• Perform additional diagnostic tests when ordered by the member’s health plan, such as diabetic eye exams or spirometry.
• Use an iPad and the clinical workflow for each evaluation.
• Provide Signify Health services as indicated, including Diagnostic & Preventive Services products such as spirometry, diabetic eye exams, and bone density screening.
• Conduct virtual visits as needed.
• Travel locally and within licensed states as needed.
• Fulfill the requirements of the specified clinician role and contribute to efforts to improve health outcomes and connect people with appropriate care.
