About The Position

Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations for health plan members. Clinicians engage with members in their homes, build trusted relationships, help bridge gaps in care, and provide flexible, comprehensive, personalized health evaluations. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and clinical workflow and are not required to prescribe medication, order lab tests, or alter a member’s current treatment regimen. The role may also include virtual visits and providing diagnostic and preventive services. The primary workplace is members’ private residences, which may contain pets, secondhand smoke, or other airborne allergens or irritants.

Requirements

Hold active, unrestricted license(s) in the coverage area(s).
Board certification is required.
Be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Nice To Haves

Multi-state licenses are preferred.

Responsibilities

Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
Build trusted relationships with health plan members and help connect them with care suited to their needs.
Provide additional diagnostic tests when ordered by a member’s health plan, including services such as diabetic eye exams, spirometry, and bone density screening.
Use an iPad and the clinical workflow for each evaluation.
Conduct virtual visits as needed.
Provide Signify Health services as indicated, including Diagnostic & Preventive Services products.
Travel locally and within licensed states as needed.
