About The Position

Signify Health, part of CVS Health, is seeking a per diem Nurse Practitioner or Physician Assistant to provide In-Home Health Evaluations to health plan members in their homes. The approximately 45-minute visits include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or change a member’s current treatment regimen. Virtual visits may also be conducted as needed.

The position focuses on building trusted relationships, helping members understand their health, and connecting them with care suited to their needs. Clinicians may provide Diagnostic & Preventive Services, such as spirometry, diabetic eye exams, and bone density screenings, and must be flexible to travel locally and within licensed states as needed.

The primary workplace is members’ private residences. Home environments vary and may include household pets, secondhand smoke, or other airborne allergens or irritants. Candidates must be able to perform their duties comfortably and safely in these conditions. This part-time, per diem position is anticipated to involve 12 hours per week.

Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.
• Ability to comfortably and safely work in homes that may contain household pets, secondhand smoke, or other potential airborne allergens or irritants.

Responsibilities

• Conduct In-Home Health Evaluations, including medication and medical history reviews and physical evaluations.
• Provide additional diagnostic tests when ordered by the member’s health plan.
• Provide Signify Health services as indicated, including Diagnostic & Preventive Services such as spirometry, diabetic eye exams, and bone density screening.
• Conduct virtual visits as needed.
• Travel locally and within licensed states as needed.
• Engage with members, answer health questions, provide education, and help connect them to appropriate care.
