About The Position

Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to provide In-Home Health Evaluations in Minneapolis. Clinicians engage with health plan members in their homes, build trusted relationships, help identify gaps in care, and connect people with care suited to their needs. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan, such as diabetic eye exams or spirometry. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or change members’ current treatment regimens. Virtual visits may also be conducted as needed. The primary workplace is members’ private residences. Home environments vary and may include household pets, secondhand smoke, or other airborne allergens or irritants; clinicians must be able to perform their duties safely and comfortably in these conditions. Local travel and travel within licensed states may be required.

Requirements

Active, unrestricted license(s) in the coverage area(s).
Board certification.

Responsibilities
