About The Position

Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to provide In-Home Health Evaluations to health plan members in Minneapolis. The role focuses on engaging with people in their homes, building trusted relationships, helping bridge gaps in care, and connecting members with care suited to their needs. Visits last approximately 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or change a member’s current treatment regimen. Virtual visits may also be conducted as needed. The primary workplace is members’ private residences. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants; clinicians must be able to perform their duties comfortably and safely in these conditions. Local and in-state travel may be needed. The position is full time, with 40 anticipated weekly hours.

Requirements

Active, unrestricted license(s) in the coverage area(s).
Board certification.
Ability to comfortably and safely perform duties in members’ homes, which may contain household pets, secondhand smoke, or other airborne allergens or irritants.

Responsibilities

Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
Perform additional diagnostic and preventive services when indicated, such as spirometry, diabetic eye exams, and bone density screening.
Use an iPad and the clinical workflow for each evaluation.
Engage with members, answer health questions, provide education, and help connect them with appropriate care.
Conduct virtual visits as needed.
Travel locally and within licensed states as needed.
Provide Signify Health services as indicated.
