About The Position

Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations in Minneapolis. The clinician visits health plan members in their homes to build trusted relationships, help identify gaps in care, and connect people with appropriate care. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or change a person’s current treatment regimen.

The role involves local travel and travel within licensed states as needed, as well as virtual visits when needed. The primary workplace is members’ private residences, which may contain household pets, secondhand smoke, or other airborne allergens or irritants. Candidates must be able to perform their duties comfortably and safely in these varied home environments.

This full-time position is scheduled for 40 hours per week. The typical pay range is $87,035 to $187,460. The actual offer depends on factors including experience, education, geography, and other relevant factors.

Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.
• Ability to comfortably and safely perform duties in members’ homes, which may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Perform additional diagnostic tests when ordered by a health plan, including services such as spirometry, diabetic eye exams, and bone density screening.
• Build trusted relationships with health plan members and help connect them to care suited to their needs.
• Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products.
• Conduct virtual visits as needed.
• Travel locally and within licensed states as needed.
• Use an iPad and clinical workflow for evaluations.
