About The Position

Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations and help bridge gaps in care. Clinicians visit health plan members in their homes, build trusted relationships, and provide flexible, comprehensive, and personalized evaluations. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or alter a member’s current treatment regimen. Virtual visits may also be conducted as needed.

The primary workplace is members’ private residences. Home environments vary and may include household pets, secondhand smoke, or other airborne allergens or irritants. The position requires the ability to comfortably and safely perform duties in these environments. The anticipated schedule is 29.5 hours per week. The typical pay range is $41.85–$90.13 per hour, with the actual offer depending on experience, education, geography, and other factors.

Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.

Responsibilities

• Conduct in-home health evaluations and build trusted relationships with health plan members.
• Review members’ medications and medical histories and perform physical evaluations.
• Provide additional diagnostic tests when ordered by the member’s health plan, such as diabetic eye exams, spirometry, and bone density screening.
• Use an iPad and the clinical workflow for each evaluation.
• Conduct virtual visits as needed.
• Provide Signify Health services, including Diagnostic & Preventive Services products as indicated.
• Travel locally and within licensed states as needed.
• Work as a clinician colleague and fulfill the requirements of the specified role.
