In-Home Nurse Practitioner or Physician Assistant Part Time - Las Vegas
About The Position
Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide In-Home Health Evaluations in members’ homes in the Las Vegas/Clark County area. Visits last about 45 minutes and include a one-on-one conversation, medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or alter members’ current treatment regimens. The clinician helps connect members with appropriate care and provides flexible, comprehensive, personalized health evaluations. The role may involve local travel and travel within licensed states as needed, as well as virtual visits. The primary workplace is members’ private residences, which may contain household pets, secondhand smoke, or other airborne allergens or irritants. Clinicians must be able to perform their duties comfortably and safely in these environments. The position is part time, with 29.5 anticipated weekly hours. Part-time employees qualify for select benefits, malpractice insurance coverage during Signify Health-related activities, supplies, and other perks. The stated pay range is $41.85–$90.13 per hour.
Requirements
- Active, unrestricted license(s) in the coverage area(s).
- Board certification.
- Able to cover the Las Vegas/Clark County primary locations.
- Able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, or other potential airborne allergens or irritants.

Responsibilities
- Conduct in-home health evaluations, including a medication and medical history review and a physical evaluation.
- Perform additional diagnostic tests when ordered by the member’s health plan, such as diabetic eye exams or spirometry.
- Build trusted relationships with members and help connect them to care suited to their needs.
- Travel locally and within licensed states as needed.
- Conduct virtual visits as needed.
- Provide Signify Health services as indicated, including Diagnostic & Preventive Services products.
