About The Position

Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to conduct in-home health evaluations and help bridge gaps in care. Clinicians build trusted relationships with health plan members and provide flexible, comprehensive, and personalized evaluations in members’ homes. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or change a person’s current treatment regimen. The role may also involve virtual visits and local travel within licensed states. The primary coverage location listed is Otero County. The primary workplace is members’ private residences, which may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Provide additional Diagnostic & Preventive Services, such as spirometry and diabetic eye exams, when indicated.
• Build trusted relationships with health plan members and help connect them to care suited to their needs.
• Conduct virtual visits as needed.
• Travel locally and within licensed states as needed.
• Fulfill the requirements of the specified clinician role and provide all indicated Signify Health services.
