About The Position

Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations to health plan members in Pueblo County. Clinicians visit members in their homes to build trusted relationships, help identify connections to appropriate care, and support improved health outcomes. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. The role does not require prescribing medicine, ordering lab tests, or changing members’ current treatment regimens. The primary workplace is members’ private residences, whose conditions vary and may include household pets, secondhand smoke, or other airborne allergens or irritants. The position includes local travel and travel within licensed states as needed, as well as virtual visits when needed. The clinician may provide Diagnostic & Preventive Services such as spirometry and diabetic eye exams.

Requirements

Active, unrestricted license(s) in the coverage area(s).
Board certification.

Responsibilities
