In-Home Nurse Practitioner or Physician Assistant Full Time - Pueblo, CO
About The Position
Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations for health plan members. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, time to answer health questions, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or change members’ current treatment regimens. The clinician builds trusted relationships, helps connect members with appropriate care, and provides personalized health evaluations in members’ homes. The role may also include virtual visits and local travel within licensed states. The primary workplace is members’ private residences, which may contain household pets, secondhand smoke, or other airborne allergens or irritants; candidates must be able to perform their duties comfortably and safely in these environments.
Requirements
- Active, unrestricted license(s) in the coverage area(s).
- Board certification.
- Ability to comfortably and safely perform duties in members’ homes, which may contain household pets, secondhand smoke, or other airborne allergens or irritants.

Responsibilities
- Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
- Answer members’ health questions and provide health education during visits.
- Perform additional diagnostic and preventive services when indicated, such as spirometry and diabetic eye exams.
- Build trusted relationships with members and help connect them to care suited to their needs.
- Conduct virtual visits as needed.
- Travel locally and within licensed states as needed.
- Fulfill the requirements of the specified clinician role and provide Signify Health services as indicated.
