About The Position

Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations to health plan members. Clinicians conduct visits lasting about 45 minutes, which include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or change members’ current treatment regimens. The role may include virtual visits and local travel within licensed states as needed. The primary workplace is members’ private residences, which may contain household pets, secondhand smoke, or other airborne allergens or irritants. The position is anticipated to require 29.5 hours per week and is eligible for select benefits, malpractice insurance coverage during Signify Health-related activities, supplies, and other perks.

Requirements

• Active, unrestricted license(s) in coverage area(s).
• Board certification.
• Able to cover Otero County.
• Able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, or other potential airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations and build trusted relationships with health plan members.
• Review members’ medications and medical histories and perform physical evaluations.
• Perform additional diagnostic and preventive services, such as spirometry or diabetic eye exams, when indicated or ordered by the health plan.
• Use an iPad and the clinical workflow for each evaluation.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide all Signify Health services indicated for the role.
• Help connect members with care suited to their individual needs.
