About The Position

As a Primary Care Advanced Practice Provider (APRN), the Nurse Practitioner delivers comprehensive, patient-centered care across the continuum in collaboration with physicians and an interdisciplinary care team. The role manages acute, chronic, and palliative care needs; conducts comprehensive assessments; develops individualized care plans; diagnoses and treats conditions; interprets diagnostic results; prescribes medications; and coordinates services to support patient outcomes. The Nurse Practitioner provides timely, evidence-based care and builds trusted relationships with patients, families, and caregivers. The position practices at the top of the clinician’s license and supports quality outcomes through care gap closure, HCC documentation, chronic disease management, and preventive care initiatives. The Nurse Practitioner acts as a patient advocate and clinical leader, coordinating with primary care providers, specialists, home health services, therapy providers, and community resources. The role also supports advanced care planning and symptom management within a value-based care model focused on improved outcomes, patient experience, and reduced unnecessary utilization. Timely documentation, accurate billing, and effective care-team communication are essential. The role manages 12–16 patients per day and participates in a rotating after-hours on-call team. The position is based at 7135 West Sahara Ave, Suite 100, Las Vegas, NV 89117. Vera Whole Health describes its care model as offering longer patient visits, collaborative care, and a focus on quality outcomes rather than volume.
