In-Home Nurse Practitioner or Physician Assistant (Part Time) - Las Vegas
About The Position
Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations to health plan members in Las Vegas. Clinicians visit members in their homes to build trusted relationships, help improve health outcomes, and connect people with care suited to their needs. Visits last about 45 minutes and include time for one-on-one conversation and questions, a medication and medical history review, and a physical evaluation. Additional diagnostic tests may be performed when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow and are not required to prescribe medication, order lab tests, or change a member’s treatment regimen. The role may also involve virtual visits and locally or in-state travel as needed. The primary work setting is members’ private residences, which may contain household pets, secondhand smoke, or other airborne allergens or irritants. Requirements
- Active, unrestricted license(s) in the coverage area(s).
- Board certification.

Responsibilities
- Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
- Engage with health plan members, answer health questions, and help connect them with appropriate care.
- Provide additional diagnostic and preventive services as indicated, such as spirometry, diabetic eye exams, and bone density screening.
- Conduct virtual visits as needed.
- Travel locally and within licensed states as needed.
- Fulfill the requirements of the clinician role and provide Signify Health services as indicated.
