About The Position

Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide In-Home Health Evaluations for health plan members. Clinicians visit members in their homes to build trusted relationships, help bridge gaps in care, and support better health outcomes. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and any additional diagnostic tests ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or alter a member’s current treatment regimen.

The primary workplace is the private residence of health plan members. Home environments vary and may contain household pets, secondhand smoke, and other potential airborne allergens or irritants; clinicians must be able to perform their duties comfortably and safely in these conditions. The role may also involve local travel and travel within licensed states, as needed, as well as virtual visits.

This part-time position is scheduled for 29.5 anticipated weekly hours. The stated pay range is $46.03–$99.14 per hour, with the actual offer depending on factors including experience, education, and geography. Requirements

• Active, unrestricted license(s) in the coverage area(s): Olympia/Thurston, Washington.
• Board certification.

Responsibilities

• Conduct In-Home Health Evaluations, including medication and medical history reviews and physical evaluations.
• Provide additional diagnostic and preventive services when indicated, such as spirometry, diabetic eye exams, and bone density screening.
• Build trusted relationships with health plan members and help connect them with appropriate care for their individual needs.
• Use an iPad and the clinical workflow for each evaluation.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide all Signify Health services indicated for the role.
