In-Home Nurse Practitioner or Physician Assistant Part Time -Olympia, WA
About The Position
Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations in the Olympia, Washington coverage area. Clinicians visit health plan members in their homes to build trusted relationships, assess health needs, and help connect members to appropriate care. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and a clinical workflow and are not required to prescribe medication, order lab tests, or change members’ current treatment regimens. The role involves local travel and travel within licensed states as needed, as well as virtual visits when needed. The primary workplace is members’ private residences, which may contain household pets, secondhand smoke, airborne allergens, or other irritants. The position is part-time, with anticipated hours of 29.5 per week. Requirements
- Active, unrestricted license(s) in the coverage area(s) of Olympia/Thurston, Washington.
- Board certification.

Responsibilities
- Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
- Perform additional diagnostic and preventive services when indicated, such as spirometry, diabetic eye exams, and bone density screening.
- Build trusted relationships with health plan members and help connect them to care suited to their needs.
- Conduct virtual visits as needed.
- Travel locally and within licensed states as needed.
- Fulfill the requirements of the specified clinician role and provide Signify Health services as indicated.
