About The Position

Signify Health, part of CVS Health, is seeking a per diem Nurse Practitioner or Physician Assistant to provide In-Home Health Evaluations to health plan members in their homes and help bridge gaps in care. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or alter a member’s current treatment regimen.

The role involves building trusted relationships, helping members connect with appropriate care, and providing Signify Health services, including Diagnostic & Preventive Services such as spirometry, diabetic eye exams, and bone density screening. Clinicians may conduct virtual visits and must be flexible to travel locally and within licensed states as needed. The primary workplace is members’ private residences, which may contain pets, secondhand smoke, or other airborne allergens or irritants. The position is part-time and per diem, with an anticipated 12 weekly hours.

Requirements

• Active, unrestricted license(s) in coverage area(s).
• Board certification.
• Ability to comfortably and safely perform duties in members’ homes, which may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

• Conduct In-Home Health Evaluations with health plan members.
