About The Position

Signify Health, part of CVS Health, is seeking a per diem Nurse Practitioner or Physician Assistant to provide in-home health evaluations in Dayton, Ohio. The clinician visits health plan members in their homes to help bridge gaps in care and provide flexible, comprehensive, and personalized health evaluations. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or change a member’s current treatment regimen.

The position involves local travel and travel within licensed states as needed, as well as virtual visits when needed. The clinician’s primary workplace is members’ private residences, which may contain household pets, secondhand smoke, or other airborne allergens or irritants. Per diem employees receive an hourly base rate plus an additional incentive structure, malpractice coverage for Signify Health-related activities, and supplies and other perks.

Requirements

• Active, unrestricted license(s) in the coverage area(s), including Dayton, Ohio.
• Board certification.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Perform additional diagnostic tests when ordered by a member’s health plan, such as diabetic eye exams, spirometry, or bone density screening.
• Use an iPad and clinical workflow to complete evaluations.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide Signify Health services as indicated, including Diagnostic & Preventive Services products.
• Build trusted relationships with health plan members and help connect them to care suited to their needs.
