In-Home Nurse Practitioner or Physician Assistant Per Diem - Dayton, OH
About The Position
Signify Health, part of CVS Health, is seeking a per diem Nurse Practitioner or Physician Assistant to provide in-home health evaluations to health plan members in Dayton, Ohio. Clinicians build trusted relationships, help members understand their health, and connect them with appropriate care. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and clinical workflow and are not required to prescribe medication, order lab tests, or change a member’s current treatment regimen. Virtual visits may also be conducted as needed. The primary workplace is members’ private residences, which may contain household pets, secondhand smoke, or other airborne allergens or irritants. The role requires local travel and travel within licensed states as needed. The position is part-time and per diem, with an hourly base rate plus possible incentives, malpractice coverage for Signify Health-related activities, supplies, and other perks.
Requirements
- Active, unrestricted license(s) in the Dayton, Ohio coverage area.
- Board certification.
- Ability to safely and comfortably perform duties in members’ homes, which may contain household pets, secondhand smoke, or other airborne allergens or irritants.

Responsibilities
- Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
- Perform additional diagnostic and preventive services when indicated, such as diabetic eye exams, spirometry, and bone density screening.
- Use an iPad and the clinical workflow for each evaluation.
- Conduct virtual visits as needed.
- Travel locally and within licensed states as needed.
- Help members understand their health and connect them with appropriate care.
