About The Position

Signify Health, part of CVS Health, is seeking a per diem Nurse Practitioner or Physician Assistant to provide In-Home Health Evaluations. Clinicians meet with people in their homes to build trusted relationships, help bridge gaps in care, and provide flexible, comprehensive, personalized health evaluations. Visits last approximately 45 minutes and include time for one-on-one connection and questions, medication and medical history review, and a physical evaluation. Additional diagnostic tests may be performed when ordered by the person’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or alter a person’s current treatment regimen.

The clinician’s primary workplace is the private residence of health plan members. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants. Clinicians must be able to perform their duties comfortably and safely in these environments. The role may also include local travel and travel within licensed states, as needed, and virtual visits.

Per diem employees receive an hourly base rate plus an additional incentive structure, such as payment per completed visit; other incentives may apply. Malpractice insurance is provided during Signify Health-related activities, along with supplies and other perks. This part-time position is eligible for a range of benefits and programs, depending on hours worked and eligibility, including dental and vision coverage, wellness resources, employee discounts, access to certain voluntary benefits, and other programs.

Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.
• Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Answer health questions and provide time for one-on-one engagement with health plan members.
• Perform additional diagnostic tests when ordered by the person’s health plan, including services such as diabetic eye exams, spirometry, or bone density screening.
• Use an iPad and the clinical workflow for each evaluation.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide Signify Health services as indicated, including Diagnostic & Preventive Services products.
• Help connect people with appropriate care and contribute to improved health outcomes.
