About The Position

Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations and help bridge gaps in care. Clinicians visit health plan members in their homes, building trusted relationships and providing flexible, comprehensive, personalized evaluations. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and any additional diagnostic tests ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or change a member’s current treatment regimen. The role may also include virtual visits and travel locally and within licensed states as needed. The primary workplace is the private residence of health plan members. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants; candidates must be able to perform their duties comfortably and safely in these conditions.

Requirements

Active, unrestricted license(s) in coverage area(s).
Board certification.
Ability to comfortably and safely perform duties in home environments that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

Provide in-home health evaluations for health plan members.
Review members’ medications and medical history and conduct physical evaluations.
Perform additional diagnostic tests when ordered by the member’s health plan, including services such as diabetic eye exams, spirometry, and bone density screening.
Use an iPad and clinical workflow to complete evaluations.
Build trusted relationships, answer health questions, and help connect members to care suited to their needs.
Travel locally and within licensed states as needed.
Conduct virtual visits as needed.
Provide Signify Health services as indicated, including Diagnostic & Preventive Services products.
