In-Home Physician Part Time - Duluth, WI
About The Position
Signify Health, part of CVS Health, is seeking a part-time Physician to provide in-home health evaluations and help bridge gaps in care. Physicians engage with people in their homes and provide flexible, comprehensive, personalized health evaluations. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and any additional diagnostic tests ordered by the person’s health plan. Physicians use an iPad and a clinical workflow. They are not required to prescribe medicine, order lab tests, or change a person’s current treatment regimen. The role involves local and in-state travel as needed, virtual visits as needed, and providing Signify Health services, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening. The primary workplace is members’ private residences, which may contain household pets, secondhand smoke, or other airborne allergens or irritants. The position is part-time, with 29.5 anticipated weekly hours.
Requirements
- Active, unrestricted physician license(s) in coverage area(s).
- Ability to comfortably and safely perform duties in residences that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities
- Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
- Perform additional diagnostic tests when ordered by a person’s health plan, and provide indicated diagnostic and preventive services.
- Use an iPad and the clinical workflow for each evaluation.
- Travel locally and within licensed states as needed.
- Conduct virtual visits as needed.
- Fulfill the requirements of the specified clinician role and contribute to efforts to improve health outcomes and connect people with appropriate care.
