About The Position

Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations and help bridge gaps in care. Clinicians engage with health plan members in their homes, building trusted relationships and helping connect people to care that meets their needs.

Visits last about 45 minutes and include time for one-on-one conversation and health questions, a medication and medical history review, and a physical evaluation. Depending on the health plan, visits may also include diagnostic tests such as diabetic eye exams or spirometry. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or change a member’s current treatment regimen.

The primary workplace is the private residence of health plan members. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants. Clinicians must be able to perform their duties safely and comfortably in these environments.

Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Answer members’ health questions and provide education during visits.
• Provide connections to appropriate care based on members’ individual needs.
• Conduct virtual visits as needed.
• Travel locally and within licensed states as needed.
• Provide Signify Health services as indicated, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening.
• Use an iPad and the clinical workflow for each evaluation.
