In-Home Nurse Practitioner or Physician Assistant Full Time - Charleston (Jackson County)
About The Position
Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations to health plan members in Charleston (Jackson County). The clinician visits members in their homes to build trusted relationships, help improve health outcomes, answer health questions, and connect people with care suited to their needs. Visits last about 45 minutes and include medication and medical history review and a physical evaluation. Additional diagnostic tests may be performed when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or change a member’s current treatment regimen. The role involves local travel and travel within licensed states as needed, as well as virtual visits when needed. The primary workplace is members’ private residences. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants; clinicians must be able to perform duties comfortably and safely in these conditions.
Requirements
- Active, unrestricted license(s) in the coverage area(s).
- Board certification is required.
- Ability to comfortably and safely perform duties in home environments that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities
- Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
- Perform additional diagnostic tests when ordered by the member’s health plan, including services such as diabetic eye exams, spirometry, and bone density screening.
- Use an iPad and the clinical workflow for each evaluation.
- Build trusted relationships with members and help connect them to care suited to their needs.
- Travel locally and within licensed states as needed.
- Conduct virtual visits as needed.
- Provide Signify Health services, including Diagnostic & Preventive Services products, as indicated.
