About The Position

Signify Health, part of CVS Health, is hiring a full-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations in Quincy, Illinois. The clinician visits health plan members in their homes to build trusted relationships, help improve health outcomes, and connect people with care suited to their needs. Visits last approximately 45 minutes and include time for one-on-one conversation, answering health questions, reviewing medications and medical history, and conducting a physical evaluation. Additional diagnostic tests, such as diabetic eye exams or spirometry, may be performed when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. The role does not require prescribing medication, ordering lab tests, or changing a member’s current treatment regimen. Virtual visits may be conducted as needed, and the clinician must be flexible to travel locally and within licensed states as needed. The primary workplace is the private residence of health plan members; home environments vary and may include household pets, secondhand smoke, or other airborne allergens or irritants. Requirements

Active, unrestricted license(s) in the coverage area(s).
Board certification.

Responsibilities

Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
