About The Position

Signify Health, part of CVS Health®, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with individuals in their homes to bridge gaps in care and build trusted relationships to improve health outcomes. The clinician will bring flexible, comprehensive, and personalized health evaluations to people's homes. The visits, lasting approximately 45 minutes, include a medication and medical history review, a physical evaluation, and potentially diagnostic tests. Clinicians will use an iPad with an intuitive workflow and are not required to prescribe medication, order lab tests, or alter current treatment regimens. The primary workplace is the private residence of health plan members, which can be diverse and may contain household pets, secondhand smoke, and other airborne allergens or irritants. Candidates must be able to perform duties safely in these environments.

Requirements

Active, unrestricted license(s) in coverage area(s)
Board certification (required)
Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants.

Responsibilities

Conduct In-Home Health Evaluations.

Conduct virtual visits as needed.
Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.).
Travel locally and within licensed states, as needed.
