About The Position

Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide In-Home Health Evaluations and help bridge gaps in care. Clinicians engage with health plan members in their homes, building trusted relationships and offering personalized health evaluations. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and, when ordered by the member’s health plan, additional diagnostic tests such as diabetic eye exams or spirometry. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or alter a member’s current treatment regimen.

The role involves local travel and travel within licensed states as needed, as well as virtual visits when needed. The primary workplace is members’ private residences, which may contain pets, secondhand smoke, and other airborne allergens or irritants; candidates must be able to perform their duties comfortably and safely in these environments.

The position is part time, with 29.5 anticipated weekly hours. Part-time employees qualify for select benefits, malpractice insurance coverage during Signify Health-related activities, supplies, and other perks. The stated pay range is $37.66–$81.11 per hour; the actual offer depends on factors including experience, education, and geography. The position is eligible for benefits and programs that may include dental, vision, wellness resources, employee discounts, and certain voluntary benefits, depending on hours worked and eligibility.

Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.
• Ability to comfortably and safely perform duties in members’ homes, which may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Provide additional diagnostic and preventive services when indicated, such as spirometry, diabetic eye exams, and bone density screening.
• Use an iPad and the clinical workflow for evaluations.
• Build trusted relationships with health plan members and help connect them to care suited to their needs.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Fulfill the requirements of the specified clinician role and provide Signify Health services as indicated.
