About The Position

Signify Health, part of CVS Health, is seeking a per diem Nurse Practitioner or Physician Assistant to provide in-home health evaluations in La Crosse. Clinicians engage with health plan members in their homes, build trusted relationships, help identify gaps in care, and connect people with care suited to their needs. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, time to answer health questions, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or change a member’s current treatment regimen. The role may also include virtual visits and local travel within licensed states. The primary workplace is members’ private residences, which may contain household pets, secondhand smoke, and other potential airborne allergens or irritants. The position is per diem and part time, with an anticipated 12 hours per week. Compensation includes an hourly base rate and may include incentives for completed visits. Malpractice insurance coverage applies during Signify Health-related activities; supplies and other perks are also provided.

Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.
• Ability to safely and comfortably perform duties in members’ homes, which may contain pets, secondhand smoke, or other potential airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Answer members’ health questions and provide education during visits.
• Perform additional diagnostic tests when ordered by a member’s health plan, including services such as spirometry, diabetic eye exams, and bone density screening.
• Use an iPad and the clinical workflow for each evaluation.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide Signify Health services as indicated, including Diagnostic & Preventive Services products.
• Help connect members with appropriate care and contribute to improving health outcomes.
