About The Position

Signify Health, part of CVS Health, is seeking a per diem Nurse Practitioner or Physician Assistant to provide In-Home Health Evaluations in members’ homes and help bridge gaps in care. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, time to answer health questions, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and a clinical workflow. They are not required to prescribe medication, order lab tests, or alter a member’s current treatment regimen.

The clinician’s primary workplace is the private residence of health plan members. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants. The role includes local travel and travel within licensed states as needed, as well as virtual visits as needed. The position is part-time and per diem, with an hourly base rate and additional incentives that may apply.

Per diem employees receive malpractice insurance coverage during Signify Health-related activities, supplies, and other perks. Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.

Responsibilities

• Conduct In-Home Health Evaluations and build trusted relationships with members.
• Review members’ medication and medical history and perform a physical evaluation.
• Answer members’ health questions and help connect them to appropriate care.
• Perform additional diagnostic tests when ordered by the member’s health plan, including services such as diabetic eye exams, spirometry, and bone density screening.
• Use an iPad and the clinical workflow for each evaluation.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide Signify Health services as indicated, including Diagnostic & Preventive Services products.
