In-Home Nurse Practitioner or Physician Assistant Part Time - Salt Lake City
About The Position
Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to conduct In-Home Health Evaluations and help bridge gaps in care. Clinicians visit health plan members in their homes, building trusted relationships and providing flexible, comprehensive, and personalized health evaluations. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and any additional diagnostic tests ordered by the member’s health plan. Clinicians use an iPad and clinical workflow and are not required to prescribe medication, order lab tests, or change members’ treatment regimens. Virtual visits may also be conducted as needed. The role involves working in members’ private residences, which may include household pets, secondhand smoke, and other airborne allergens or irritants. Clinicians must be able to perform their duties comfortably and safely in these environments. The role requires flexibility to travel locally and within licensed states as needed. Requirements
- Active, unrestricted license(s) in coverage area(s).
- Board certification.
- Ability to comfortably and safely perform duties in members’ homes, which may contain household pets, secondhand smoke, or other potential airborne allergens or irritants.

Responsibilities
- Conduct approximately 45-minute In-Home Health Evaluations, including medication and medical history reviews and physical evaluations.
- Provide additional Diagnostic & Preventive Services when indicated, such as spirometry, diabetic eye exams, and bone density screening.
- Build trusted relationships with health plan members and help connect them to appropriate care.
- Use an iPad and the clinical workflow for each evaluation.
- Travel locally and within licensed states as needed.
- Conduct virtual visits as needed.
- Fulfill the requirements of the specified clinician role and provide Signify Health services as indicated.
