About The Position

Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations in Grand Rapids. Clinicians engage with health plan members in their homes, build trusted relationships, help bridge gaps in care, and connect people with care suited to their needs. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and any additional diagnostic tests ordered by the health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or alter a member’s current treatment regimen.

The clinician’s primary workplace is members’ private residences. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants. The role may also include virtual visits and local travel, including within licensed states as needed. Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.
• Ability to comfortably and safely perform duties in varied home environments that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Perform additional diagnostic tests when ordered by a health plan, including services such as diabetic eye exams, spirometry, and bone density screening.
• Build trusted relationships with health plan members and help connect them with care suited to their needs.
• Use an iPad and the clinical workflow for each evaluation.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide Signify Health services as indicated, including Diagnostic & Preventive Services products.
