About The Position

Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide In-Home Health Evaluations to health plan members in their homes. The role focuses on building trusted relationships, helping members understand their health, and connecting them with appropriate care. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or change a member’s current treatment regimen.

The clinician may conduct virtual visits as needed and provide Signify Health services, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening. The role requires local travel and travel within licensed states as needed. The primary workplace is members’ private residences, which may contain household pets, secondhand smoke, or other airborne allergens or irritants. The position is listed at 29.5 anticipated weekly hours, with a typical pay range of $41.85–$90.13 per hour.

Requirements

• Active, unrestricted license(s) in coverage area(s).
• Board certification is required.
• Ability to comfortably and safely perform duties in members’ homes, which may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.
