About The Position

Signify Health, part of CVS Health, is seeking a per diem Nurse Practitioner or Physician Assistant to provide In-Home Health Evaluations to health plan members in their homes and help bridge gaps in care. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the person’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or alter a member’s current treatment regimen.

The clinician’s primary workplace is members’ private residences. Home environments vary and may contain household pets, secondhand smoke, or other airborne allergens or irritants. Clinicians should be able to perform their duties comfortably and safely in these settings. Local travel and travel within licensed states may be needed, and virtual visits may be conducted as needed.

This part-time position is anticipated to require 12 hours weekly. Pay is hourly, with an additional incentive structure that may include payment per completed visit. The role includes malpractice insurance coverage for Signify Health-related activities, supplies, and other perks. Depending on hours worked and eligibility, part-time benefits may include dental and vision coverage, wellness resources, employee discounts, access to certain voluntary benefits, and other programs.

Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.
• Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

• Conduct In-Home Health Evaluations, including medication and medical history reviews and physical evaluations.
• Perform additional diagnostic tests when ordered by the member’s health plan, such as diabetic eye exams, spirometry, or bone density screening.
• Use an iPad and the clinical workflow for each evaluation.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide Signify Health services as indicated, including Diagnostic & Preventive Services products.
• Engage with members, answer health questions, and help connect them to care suited to their needs.
