In Clinic Visit Facilitator- Primary Care Clinic
About The Position
Fairview is hiring an In Clinic Visit Facilitator for its Primary Care team in Chisago City, Minnesota. This is a 0.65 FTE position (52 hours per two-week pay period) and is benefits-eligible. Clinic hours are Monday through Thursday, 7:00 a.m. to 6:00 p.m., and Friday, 7:00 a.m. to 5:00 p.m. The position does not require weekend work and may require travel to the North Branch clinic to support business needs. The Visit Facilitator supports in-person clinic visits by communicating with patients before appointments, guiding them through the care experience, gathering pertinent medical information, and documenting it in the patient’s medical record. The role also helps keep exam rooms stocked and supplies and equipment available, and performs tasks that support the team’s effectiveness and efficiency. Additional duties may include scheduling and coordinating appointments, preparing schedules, and other delegated responsibilities.
Requirements
- At least 1 year of customer service experience.
- Proficiency with technology, including computer hardware and programs and smart devices such as phones or tablets.
Nice To Haves
- Current student in nursing, medicine, social work, psychology, or pharmacy.
- Successful completion of a Nursing Assistant, Emergency Medical Technician, or Health Unit Coordinator program.
- Knowledge of medical terminology.
- Experience with Epic Health Records.
- Telephonic customer experience.
- Previous experience working in a healthcare setting.
Responsibilities
- Communicate with patients before appointments as needed to ensure applicable medical records are collected, received, and available for the visit.
- Assist with rooming patients for in-person visits by collecting and documenting the reason for the visit, patient-reported medication information, vitals using an automated vitals machine, and applicable health questionnaires.
- Prepare exam rooms and stock supplies.
- Support patients’ connection to the provider and help ensure a positive patient-provider experience.
- Complete visit wrap-up by providing the after-visit summary and ensuring patients understand follow-up recommendations; assist with scheduling and coordinating follow-up appointments as needed.
- Communicate effectively with patients, providers, and other health professionals, both in person and using virtual technologies.
- Assist with scheduling and coordinating appointments, preparing schedules, and other delegated duties as assigned.
- Travel to the North Branch clinic as needed to support business needs.
