About The Position

Project Renewal is a New York City nonprofit organization focused on building supportive communities and helping people achieve dignity and independence through health, housing, and employment. Its integrated programs operate 24/7. The Housing Case Manager works in one of the organization’s housing programs in the Bronx, Brooklyn, or Manhattan, providing individualized case management and supportive services to clients.

Requirements

Bachelor’s degree in Social Work or a related field.
Excellent oral, writing, and listening skills.
Computer proficiency.

Nice To Haves

1–2 years of experience working with homeless adults with mental illness or another special-needs population.
Bilingual or multilingual ability.

Responsibilities

Provide intake, psychosocial, and substance abuse assessments and develop an appropriate treatment plan for each client on the assigned caseload.
Provide case management services that support the creation of and adherence to individual Service Plans, including short- and long-term goals and objectives.
Conduct home visits and coordinate medical, psychiatric, and substance abuse services.
Monitor medication compliance as necessary.
Maintain written information and case notes about client contacts and interventions in the AWARDS/CARES system.
Conduct individual and group supportive counseling sessions.
Organize and educate tenants through groups and activities on topics of interest, such as money management and employment opportunities.
Link clients to community services.
Participate in weekly clinical meetings to discuss high-risk clients and update spreadsheets related to client progress.
Perform other duties as assigned by supervisory staff.
