About The Position

As a home care registered nurse, you provide comfort-focused nursing care to hospice patients with prognoses of six months or less and build compassionate, meaningful connections with patients and the people in their lives. You work with an interdisciplinary team of hospice clinicians to support patients and families, advocate for each patient’s goals of care, and provide basic hospice education and training to patients, families, and caregivers.

Most of your time is spent caring for patients wherever they call home, including private residences and assisted living or skilled nursing facilities. You typically make 4–6 visits each day. Once a week, you visit the local program office to meet with an interdisciplinary team—including social workers, physicians, chaplains, and aides—who collaborate to provide holistic support.

Requirements

• Associate or bachelor's degree in nursing.
• Valid RN license.
• At least one year of hospital nursing experience.
• Reliable transportation.

Responsibilities

• Provide comfort-focused in-home nursing care for patients with prognoses of six months or less.
• Work with an interdisciplinary hospice team that includes LPNs, aides, chaplains, social workers, and physicians.
• Advocate for patients and families, ensuring each patient’s goals of care are communicated and prioritized by the care team.
• Provide basic hospice education and training to patients, families, and caregivers.
• Visit patients in private residences and assisted living or skilled nursing facilities.
• Attend a weekly meeting at the local program office with the interdisciplinary team.
