About The Position

As a home care registered nurse, you provide comfort-focused in-home nursing care to hospice patients with prognoses of six months or less. You work with an interdisciplinary team of hospice clinicians and advocate for patients and families, ensuring their goals of care are communicated and prioritized. You also provide basic hospice education and training to patients, families, and caregivers. Most care takes place where patients call home, including private residences and assisted living or skilled nursing facilities. You typically make 4–6 visits each day and visit the local program office once a week to meet with an interdisciplinary team that collaborates to support patients and families.

Requirements

Associate or bachelor's degree in nursing.
Valid RN license.
At least one year of hospital nursing experience.
Reliable transportation.

Responsibilities

Provide comfort-focused in-home nursing care for patients with prognoses of six months or less.
Work with an interdisciplinary team of hospice clinicians, including LPNs, aides, chaplains, social workers, and physicians.
Advocate for patients and families and ensure their goals of care are communicated and prioritized by the care team.
Provide basic hospice education and training to patients, families, and caregivers.
Make typically 4–6 visits each day in patients’ homes or care facilities.
Visit the local program office once a week to meet with the interdisciplinary team.
