About The Position

Residential Home Health and Hospice is hiring a Registered Nurse to provide end-of-life care to patients in their homes in the Elgin, Illinois area. The RN delivers patient-centered hospice care focused on comfort, dignity, and quality of life while collaborating with an interdisciplinary team. The position offers independent field work with support from a dedicated field education team and leadership. It is eligible for a $10,000 first-year success bonus tied to key milestones.

Requirements

Active Registered Nurse (RN) license in the state where the employee works.
Two years of RN experience and clinical expertise to deliver quality patient care.
Valid driver’s license.
Ability to spend approximately 20% of the workday driving to and from patient locations.
Successful completion of a drug screen prior to employment.
Legal authorization to work in the United States.

Responsibilities

Deliver compassionate, patient-centered care in a hospice setting in collaboration with an interdisciplinary team.
Perform comprehensive patient assessments, symptom and pain management, and individualized end-of-life care planning.
Educate and support patients and families throughout the end-of-life journey.
Think critically and solve problems in real time.
Complete thorough, compliant electronic medical record documentation.
Travel to patient homes within the assigned territory.
