Hospice Registered Nurse (RN)
About The Position
As a home care registered nurse, you will provide comfort-focused in-home nursing care to hospice patients with prognoses of six months or less. You will build compassionate, meaningful connections with patients and the people in their lives, advocate for each patient and family, and help ensure their goals of care are communicated and prioritized by the care team. You will also provide basic hospice education and training to patients, families, and caregivers. The role works with an interdisciplinary team of hospice clinicians, including LPNs, aides, chaplains, social workers, and physicians. You will spend most of your time with patients wherever they call home, including private residences and assisted living or skilled nursing facilities, typically making four to six visits each day. Once a week, you will visit the local program office to meet with the interdisciplinary team, which collaborates to provide holistic support for patients and families. Requirements
- Associate or bachelor's degree in nursing.
- Valid RN license.
- At least one year of hospital nursing experience.
- Reliable transportation.
