As a home care registered nurse, you provide comfort-focused in-home nursing care to hospice patients with prognoses of six months or less. You build compassionate, meaningful connections with patients and the people in their lives, advocate for patients and families, and help ensure their goals of care are communicated and prioritized by the care team. You also provide basic hospice education and training to patients, families, and caregivers. You work with an interdisciplinary hospice team that includes LPNs, aides, chaplains, social workers, and physicians. Most of your time is spent caring for patients wherever they call home, including private residences and assisted living or skilled nursing facilities. You typically make 4–6 visits each day and visit the local program office once a week to meet with the interdisciplinary team.
