Hospice Registered Nurse (RN)
About The Position
As a home care registered nurse, you provide comfort-focused in-home nursing care to hospice patients with prognoses of six months or less. You work as part of an interdisciplinary hospice team and advocate for patients and families, ensuring their goals of care are communicated and prioritized. You also provide basic hospice education and training to patients, families, and caregivers. Most of your time is spent caring for patients wherever they call home, including private residences and assisted living or skilled nursing facilities. You typically make 4–6 visits each day and visit the local program office once a week to collaborate with an interdisciplinary team of social workers, physicians, chaplains, and aides. Requirements
- Associate or bachelor's degree in nursing.
- Valid RN license.
- At least one year of hospital nursing experience.
- Reliable transportation.
Responsibilities
- Provide comfort-focused in-home nursing care for hospice patients with prognoses of six months or less.
- Work with interdisciplinary hospice clinicians, including LPNs, aides, chaplains, social workers, and physicians.
- Advocate for patients and families by ensuring their goals of care are communicated and prioritized by the care team.
- Provide basic hospice education and training to patients, families, and caregivers.
- Make typically 4–6 patient visits each day.
- Visit the local program office once a week to collaborate with the interdisciplinary team.
