About The Position

Emblem Home Health & Hospice is seeking marketers in the Phoenix area, including Scottsdale/Fountain Hills, Glendale/Sun City, and Tempe/Chandler/Mesa. The Hospice Liaison serves as a trusted community resource, building relationships with healthcare providers and guiding patients and families toward hospice services. The role focuses on supporting dignity, comfort, and care during challenging moments, and works with care coordination teams to help ensure smooth patient transitions.

Requirements

Bachelor’s degree in Marketing, Business Administration, or a related field preferred.
Minimum of 3 years of healthcare marketing experience; hospice experience is strongly preferred.
Strong communication, relationship-building, and public relations skills.
Ability to work independently while maintaining a compassionate, patient-first approach.
Demonstrated organizational skills and ability to meet outreach goals.
Reliable transportation, a valid Arizona driver’s license, and proof of insurance.

Nice To Haves

Bachelor’s degree in Marketing, Business Administration, or a related field.
Hospice experience.

Responsibilities

Develop and maintain relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, and other referral sources.
Educate healthcare professionals and community groups about hospice services and benefits.
Serve as a knowledgeable, compassionate resource for patients and families during the referral and admission process.
Document outreach activities and maintain accurate records of interactions.
Ensure all messaging reflects the organization’s commitment to dignity, comfort, and quality care.
Collaborate with care coordination teams to support smooth transitions for patients.
Monitor community needs and identify opportunities to improve access to hospice care.
