About The Position

The Home Infusion Clinical Liaison drives patient recruitment, referral growth, and provider engagement for the Home Infusion service across the Boston Medical Center Health System while executing patient advocacy functions throughout the continuum of care. Operating under administrative direction as a non-management professional, this role serves as the central intermediary among referral sources, interdisciplinary clinical care teams, patients, and caregivers to accelerate awareness, utilization, and community access to specialized home infusion therapies. By executing programmatic outreach, structured clinical education, and transition-of-care coordination, the position expands systemic referral pipelines, accelerates onboarding timelines, optimizes community health outcomes, and advances organizational initiatives focused on delivering equitable, patient-centered care.

Requirements

  • Bachelor’s degree in Nursing, Healthcare Administration, Business, Marketing, Life Sciences, or a related healthcare field, combined with 3–5 years of experience in home infusion, specialty pharmacy, healthcare business development, care coordination, or clinical liaison roles, is required, or an equivalent combination of education and experience.
  • Registered Nurse (RN) license.
  • Knowledge of home infusion therapies, clinical referral requirements, and discharge planning workflows.
  • Knowledge of commercial insurance authorization processes, payer-specific criteria, and healthcare reimbursement models.
  • Skill in establishing and maintaining professional relationships with physicians, advanced practice providers, case managers, and hospital leaders.
  • Skill in delivering clinical presentations, programmatic in-services, and educational training to healthcare professionals and consumer audiences.
  • Ability to coordinate care across interdisciplinary teams, including Pharmacy, Nursing, Revenue Cycle, and Drug Access departments.
  • Ability to manage multiple referral pipelines, track data metrics, and prioritize tasks independently in a hybrid work environment.

Nice To Haves

  • Experience utilizing the Epic electronic medical record (EMR) platform is preferred.
  • Demonstrated experience in patient recruitment, provider relationship management, or healthcare referral development is preferred

Responsibilities

  • Drives referral development and business growth by establishing, maintaining, and expanding professional relationships with physicians, advanced practice providers, nursing leaders, case managers, discharge planners, home health agencies, affiliated provider networks, hospitals, and community partners to increase clinical utilization of Home Infusion services system-wide.
  • Coordinates referral intake and patient onboarding operations by reviewing, prioritizing, qualifying, and converting submitted patient referrals into active home therapy starts while collaborating with Pharmacy, Nursing, Revenue Cycle, and Drug Access teams to facilitate streamlined discharge planning and timely clinical entry.
  • Facilitates care transitions and referral source communications by acting as the primary point of contact among providers, patients, families, and interdisciplinary team members to supply continuous status updates regarding therapy initiation and systemic access barriers.
  • Supports patient and caregiver engagement initiatives by educating individuals on available treatment modalities, service expectations, home administration procedures, personal financial responsibilities, and therapy-related safety rules to encourage regulatory compliance and service retention.
  • Maintains operational documentation, data reporting, and performance improvement registries by utilizing electronic medical records, specialized tracking software, and organizational reporting tools to evaluate referral pipelines, conversion rates, and access barriers for programmatic refinement.
  • Delivers formalized clinical and programmatic instruction by conducting presentations, in-services, consultations, and structured training sessions for healthcare providers, regional referral sources, patients, and families concerning home infusion offerings, specialty pharmacy criteria, and insurance qualification frameworks.
